Complete armamentarium assembled

Position the dental equipment to facilitate treatment and ergonomics

Procedure explained to the patient

Record current oral hygiene practices in the patient chart

Use mirror to inspect oral cavity

Complete plaque indexing on patient; provide patient with hand mirror to review score

Use patient’s current oral hygiene practices and plaque index score to customize oral hygiene instruction for patient

Had patient demonstrate current at home brushing & flossing

Demonstrate tooth brushing method recommended for patient

Demonstrate flossing

Have patient demonstrate revised techniques

Discuss role of sugar exposure, acid attack and dental caries

Provide verbal summary of oral hygiene goals to patient

Procedure documented in chart

Safety measures followed

Infection control protocols followed

Demonstrates professionalism and appropriate communication skills

Patient is seated in upright position and procedure explained

Use mirror to inspect oral cavity

Appropriate sized fluoride trays used for patient

Appropriate type of fluoride used for patient

Appropriate amount of fluoride dispensed

Patient given LVE and instructed on use

All teeth dried and retraction maintained

Fluoride trays inserted with cotton roll(s) in place

Fluoride trays left in place for appropriate time

Excess fluoride removed

Complete post-operative instructions provided

Procedure accurately documented

Safety measures followed

Infection control protocols followed

Demonstrates professionalism and appropriate communication skills
