Assessment

  •  Reviewed precautions for client specific isolation category.
  • Considered types of care to be delivered to client while in the room.
  • Determined that client understands the purpose of isolation and procedures.
  • Determined if client is allergic to latex, if using latex gloves/products.

Nursing Diagnosis

  • Identified purpose and risks associated with applying protective measures.

Planning

  • Developed individualized goals for client based on nursing diagnosis and identified expected outcomes.
  • Prepared equipment and supplies, selecting correct type and size of gloves.

Evaluation

  • Determined if client has had opportunity to discuss health problems, course of treatment, and related concerns, while in isolation room.
  • Identified unexpected outcomes.
  • Documented and reported pertinent information related to client.

Personal Protective Equipment – Implementation

  1. Performed hand hygiene.
  2. Applied protective wear in order:
    1. Applied isolation gown with opening at the back and edges overlapping. Tied gown at the waist and neck.
    2. Applied mask to fit snugly over mouth and nose. Avoided touching mask while being worn.
    3. Applied eye protection/face shield when there was a potential for splashes or sprays of blood, body fluids, secretions or excretions.
    4. Applied gloves with cuffs on top of gown cuffs.
  3. Entered client’s room. Arranged supplies and equipment.
  4. Explained purpose of isolation and necessary precautions to client, family and visitors. Assess client for emotional response to isolation precautions. Allow for time for client to ask questions.
  5. Performed procedure.
  6. Restocked room as needed with another caregiver handling supplies at the door.
  7. Removed protective equipment in order:
    1. Removed gloves by grasping the outside of the palm area of one glove without touching wrist.
    2. Removed glove by turning inside-out and feeding into gloved hand.
    3. Slid 2-3 fingers of un-gloved hand inside remaining glove.
    4. Peeled second glove off, inside-out and discarded gloves.
    5. Washed hands.
    6. Removed eye protection and discarded and/or placed for cleaning.
    7. Removed gown (first untied neck strings then back strings) touched the inside of the gown, rolled (inside out) into a ball and discarded or placed in hamper.
    8. Removed mask (first untied bottom mask string) did not touch outer mask. Discarded.
    9. Washed hands.
    10. Exited room, through anteroom if attached.
  8. Did not remove any equipment from isolation room.
