As part of the Transfusion Medicine , MLT course fresh blood samples are preferred to provide students with the best learning experience in labs.

Student participation in blood collection from each other at the beginning and/or during the semester as a source of fresh samples may be required. In such cases faculty:

• Anonymizes student specimens
• Performs phenotyping/ any related testing.

All phenotyping results/findings are confidential. Student samples may become “manufactured “lab specimens (as needed) during the semester. During the semester, student samples may be part of specimen development for the transfusion labs.

I have read and understand the above information and consent to having my blood collected and used for “manufactured” specimens in Transfusion Medicine labs. I know that I can exit from this agreement at any time.
